  • Provides care for long term care, rehab care and acute changes in condition to patients in long term care including IV access, PICC lines, peripheral IV, Metaports, Colostomy care, enteral feeding care, Peritoneal Dialysis, Wound Vac system, direct care, including medication administration and treatment application and bedside patient care when necessary.
  • Performs weekly wound rounds with wound nurse and evaluate effectiveness of treatment to wounds, and pressure ulcers.
  • Assists in physician's routine patient rounds and notification of changes in condition
  • Provide patient care using the nursing process of assessments, nursing diagnosis, planning, implementation and evaluation
  • Participates in all management functions of the nursing department (planning, organizing, leading and controlling).
  • Serve as a resource to staff in clinical and managerial decision-making
  • In collaboration with the RNAC, assures patient assessments and care plans  in compliance with regulatory and other standards. 
  • Facilitates interdisciplinary communication and collaboration working closely with the Director of Nursing, including staff discipline 
  • Collaborate with the Restorative Nurse in assuring programs are being implemented and documented on correctly.
