Purpose:
Provide a centralized, personal intake process as a client service function in order to gather pertinent patient/client, clinical and financial information to optimize sales, customer responsiveness and satisfaction, as well as quality care. TCC uses telephone, physical meetings, and other follow-up techniques to convert inquiries into requests for service.
Essential Functions:
-Act as a contact liaison person interfacing with referral sources, outside agencies, health professionals and field staff.
-Receive and coordinate all incoming calls, provide patients/clients and referral sources with basic information to assist in accessing appropriate services.
-Communicate with patient/client and family regarding arrangements for the initiation of care/services.
-Complete intake screening including obtaining, documenting and analyzing all required information to make a preliminary admission decision.
-Receive and logs referrals. Prepare reports and responses to inquiries. Provides telephonic contact and system data entry for customers seeking care/services.
-Actively promote care/services to prospective patients/clients.
-Where permitted by law, receives verbal orders from physicians to initiate care/services.
-Collect and enter customer information into the management information system.
-Work in conjunction with clinical staff to adhere to standards of practice for nursing and applicable law and regulations.
-Plan activities and initiate contacts in the community to enhance the visibility of the company and generate an increasing customer base.
-Collaborate closely with Interim Healthcare's current Director of Business Development in identifying potential new opportunities and ideas for expanding existing client base.
-Actively participate as a member of a dynamic, high performance work team to effectively manage, and change business and care plans to optimize the patient/client experience.·
-Hired as a continuous care pediatric nurse. Worked 8 to 12 hour shifts in pediatric patient's homes. Provided personal care, administered medications, provided tracheostomy care, completed assessments, and met any other clinical or personal needs of the patient.
-Hired as a skilled home health visit nurse. Completed 6 to 9 skilled home health nursing visits per day. Visits included: education related to disease process and management, education related to medication administration, proper use, and side effects, wound care, and IM/SQ injections as needed. Worked closely in conjuction with physicians to be sure patient's plan of care was being followed.
