The Case Manager works closely with the insurance provider for the patient to obtain equipment and services the patient will need after discharge. The case manager will also arrange follow up appointments and medications for the patient. Case Managers insure that patients receive instructions and education from the appropriate providers before discharge.
The case manager at Providence also participates in Utilization Review when required.
