National Travel Nurse Jul 2009Present Healix Infusion,Inc Sugar Land, TX  This position requires travel weekly to assignments at physician based Infusion Centers. I assume responsibility while staff are on  vacation or leave. As an Infusion Coordinator, I administer IV therapies to patients under physicians care. Therapies include, though are not limited to,  IVAB, IVIG, Remicade, Tysabri. I maintain venous access catheters; PICCs, Implanted Ports, Hickman Catheters and place PIV when necessary. I coordinate services for patients with Home Health, hospitals and physician offices to ensure that the patient receives the care they require. 
Patient and family education is an ongoing process. Scheduling appointments to ensure smooth work flow and avoidance of long wait times and miscommunication.

Infusion Casemanager Dec 2010Aug 2013 Coram Home Infusion Mt Prospect, IL Traveling to patients home to provide services. Patients requiring infusion therapies  for acute and chronic illnesses. Approx 40%  chronic patients. Line maintenance, lab draws. All age groups. Emphasis on teaching patient and families skills needed to self administer medication when necessary, along with lifestyle adaptation. Patient population included serving clients of Cancer Treatment Centers of America.Communication with insurance case managers and physician offices as needed. Arrangements made with DME providers.
Full assessments completed and documented each visit.

Home Care Nurse in traditional setting. Adult patients post hospital stay after surgery or illness. Majority of patients elderly with chronic illnesses. Visits based on front loading and lasting approx 9 weeks, more if medicare allowed. When hired initially, I was the CNC. Duties included supervision of staff nurses, educating staff on procedures and tasks required to do their assignments. provide support clinically when needed. Placed PICC when Interventional Radiology not available. Responded to all CODE situations(early Rapid Response).
Team member of first Magnet Committee, and the hospital did obtain Magnet status.  
Staff Nurse Outpatient Infusion Center Sep 2006Feb 2008 St Joseph Medical Center Elgin, IL Per Diem position at local hospital in outpatient setting, 1-2 days a week.
 Patients seen received IVAB, blood products, chemotherapies, remicade, tysabri and hydration. Maintenance of IV lines and lab draws. 
While employed here I obtained by chemotherapy certification but did not maintain since I no longer worked there.
